    Here are the guidelines for starting the process electronically.

    Download and complete the following documents from the Forms section of this site or request them directly from JNFM:

    • Customer Information Form (CIF) Individuals – Form AO1
    • Client Investment – Form AO4
    • Two completed Character References – Form AO6
    • Certificate of Foreign Status for US Tax Withholding – W8BEN AOF
    • Request for Taxpayer ID number – W9 AOF

     

    These are the supporting documents that are required for each account holder named on the CIF:

    • Tax Registration Number (TRN Card)
    • Valid Identification (Driver’s License or Passport or National Identification Card)
    • Proof of Address (utility bill or credit card statement or bank statement)

     

    Other forms may need to be completed depending on the product and the level of service required. They include:

    • Fax and Email Indemnity – Form OF1. (Needed by persons who will need to give instructions by email or fax)
    • Master Retail Repurchase Agreement – Form AO3 (Only applicable to Repurchase Agreements)
    • LSA Declaration (for the JNFM Tax-Free Account)
    • CI Open & Registered Plan Mutual Fund Application – Form AO8 (for CI Mutual Funds)
    • Mutual Fund Declaration (for CI Mutual Funds)
    • CI Web Login Request Form (for clients who wish to view their CI accounts online)

    Persons who reside outside of Jamaica will also need the documents listed above. Documents sent from overseas should be notarized overseas. In addition, a Bank Reference Form should be completed by your bank. This form is also available on this site.

    The items noted above may be sent by hand, courier or regular mail to:

    Within three days of receiving the documents, a JNFM representative will contact you by telephone or email to advise you how to send your investment to JNFM.
